Atorvastatin is a prescription medicine used to lower LDL cholesterol, often called "bad" cholesterol, in your blood. It belongs to a group of medicines called statins.
It works in your liver, where it blocks an enzyme your body uses to make cholesterol. With less cholesterol being made, your liver takes more LDL out of your bloodstream. Over time, this helps lower your chance of a heart attack or stroke, alongside healthy eating, regular activity and not smoking.
In Malaysia, doctors prescribe atorvastatin based on your overall heart risk, not only your cholesterol number. Your age, blood pressure, diabetes, smoking and family history all count. That's why a doctor needs to assess you before you start.
Results and side effects vary from person to person.
Atorvastatin dosage comes in four strengths, grouped by how strongly they lower LDL cholesterol. Your doctor chooses the strength that suits your heart risk and cholesterol target.
Lowers LDL by a moderate amount
The lowest strength and a common starting dose. Often used to help prevent a first heart attack or stroke.
Brands: Lipitor, Zarator
Used when overall heart risk is higher, or as a step up from 10mg if cholesterol is still above target.
Brands: Lipitor, Zarator
Lowers LDL the most
Often prescribed after a heart attack or stroke, or for very high heart risk, such as diabetes with other risk factors.
Brands: Lipitor, Zarator
The highest strength, usually for established heart or blood vessel disease. Your doctor will monitor you more closely.
Lipitor only
What your doctor looks at
Never change your dose without speaking to your doctor. Results and side effects vary from person to person.
Lipitor is the originator brand of atorvastatin and Zarator is a generic. Both are made by Viatris and registered with Malaysia's National Pharmaceutical Regulatory Agency (NPRA), which checks medicines for quality, safety and efficacy.

Food, drink and follow-up
Most people take atorvastatin without problems. Like all medicines, it can cause side effects, although not everyone gets them. Side effects vary from person to person, and many settle as your body gets used to the medicine.
Tell your doctor if any of these bother you or don't go away.
Stop taking atorvastatin and contact a doctor straight away if you have:
Call 999 or go to the nearest emergency department if you have signs of a serious allergic reaction, such as swelling of your face, lips or tongue, difficulty breathing, or a widespread rash.
Some medicines can raise the level of atorvastatin in your blood and increase the chance of side effects. Tell your doctor or pharmacist if you take:
Avoid drinking large amounts of grapefruit juice while taking atorvastatin.
This is not a complete list. Read the patient information leaflet in your pack, and ask your pharmacist if you're unsure.
All three are statins and work the same way. They differ mainly in strengths and when you take them.

About atorvastatin
Atorvastatin is used to lower LDL cholesterol and reduce the chance of heart attack and stroke. Doctors prescribe it for people with high cholesterol, for people with heart or blood vessel disease, and for people whose overall heart risk is raised, for example because of diabetes. It works best alongside healthy eating, activity and not smoking.
Lipitor is a brand of atorvastatin. Atorvastatin is the active ingredient, and Lipitor is the original brand name. Other registered brands in Malaysia, such as Zarator, contain the same active ingredient.
Both contain atorvastatin and are made by Viatris. Lipitor is the originator brand and Zarator is a generic. They differ in tablet coating, appearance and price. Zarator comes in 10mg, 20mg and 40mg, while Lipitor also comes in 80mg. Speak to your doctor or pharmacist before switching between brands.
Price and buying
At Hello Doktor, a box of 30 tablets costs RM53 to RM79 for Zarator and RM129 to RM179 for Lipitor, depending on strength.
No. Atorvastatin is a prescription medicine in Malaysia. If you don't have a prescription, you can speak to a licensed doctor by video for RM1. They'll check whether atorvastatin is suitable and prescribe it if it is.
Doses
Both are moderate-intensity doses. 10mg is a common starting dose, and 20mg is used when your heart risk is higher or your cholesterol needs a bigger reduction to reach your target. Your doctor decides which strength you need.
Yes. Atorvastatin 40mg and 80mg are high-intensity doses. They're usually prescribed for people who have had a heart attack or stroke, or who have very high heart risk. Your doctor will monitor you with blood tests.
You can take atorvastatin at any time of day, with or without food. What matters most is taking it at the same time every day so you don't forget.
Side effects and safety
The most common are cold-like symptoms, headache, nausea, digestive upset, and muscle or joint aches. Many settle with time. Contact a doctor straight away if you have unexplained muscle pain or weakness, dark urine, or yellowing of your skin or eyes.
Yes, muscle aches can happen. Most are mild, but rarely statins cause serious muscle damage. Stop taking it and contact a doctor straight away if you have unexplained muscle pain, tenderness or weakness, especially with a fever or dark urine.
You can usually drink alcohol in moderation. Heavy drinking increases the chance of liver problems, so keep within recommended limits and tell your doctor if you drink regularly.
Avoid drinking large amounts of grapefruit juice. It can raise the level of atorvastatin in your blood and increase the chance of side effects.
No. Don't take atorvastatin if you're pregnant, trying to get pregnant or breastfeeding. If you become pregnant while taking it, stop and speak to your doctor.
Treatment
Atorvastatin starts lowering cholesterol within a few weeks. Your doctor will usually recheck your blood 4 to 12 weeks after you start or change dose. How much your cholesterol changes varies from person to person.
Most people take atorvastatin long term, because cholesterol usually rises again when you stop. Don't stop taking it without speaking to your doctor.
Neither is better for everyone. Both are effective statins, and the right choice depends on your cholesterol, your heart risk, other medicines you take and how you respond. Your doctor will choose with you.
