An enlarged prostate, and what dutasteride does about it
The prostate sits below the bladder and wraps around the urethra. With age it commonly enlarges without being cancerous, a condition called benign prostatic hyperplasia. As the gland grows it narrows the passage urine travels through, which is why the stream weakens and the bladder rarely feels empty.
Dutasteride blocks both type 1 and type 2 of the enzyme that converts testosterone into dihydrotestosterone, the hormone that drives prostate growth. The gland shrinks slowly, so this is a daily capsule reviewed over months rather than something that works on the day you take it.
Tamsulosin, the second ingredient in Duodart, does something different and faster. It relaxes the smooth muscle at the bladder neck and in the prostate, so the existing passage opens up. Men often notice that part within days, while the dutasteride part is still working in the background.

Taking it day to day
When to take it
- One capsule a day, at roughly the same time each day
- Duodart is taken about 30 minutes after the same meal each day, which keeps the tamsulosin level steady
- Avodart can be taken with or without food
- Swallow the capsule whole, do not chew or open it
If you miss a dose
- Take it when you remember on the same day
- If the day has passed, skip it and take the next one as usual
- Never take two capsules to catch up
- With Duodart, restarting after several missed days can bring back the dizziness of the first few doses, so stand up slowly
Handling and storage
- Store below 30C in the original pack, away from heat
- Women who are or may become pregnant should not handle leaking capsules, as dutasteride is absorbed through the skin
- Do not donate blood while taking it and for six months after the last dose
- Keep it out of reach of children
PSA testing, safety and side effects
Dutasteride halves your PSA reading
- After about six months, dutasteride lowers serum PSA by roughly half in men without prostate cancer
- Tell any doctor who orders a PSA test that you take dutasteride, so the result is doubled before it is interpreted
- A PSA that keeps rising while you are on dutasteride needs assessment, even if the absolute number still looks low
- Dutasteride does not remove the need for prostate cancer screening decisions with your doctor
Reported side effects
The first four are reported with dutasteride itself, so they apply to both products. The last three come from the tamsulosin in Duodart and are not expected with Avodart.
- Reduced sex drive
- Difficulty getting or keeping an erection
- Reduced volume of semen
- Breast tenderness or enlargement
- Dizziness or light headedness on standing, more likely in the first days of Duodart
- Ejaculation problems, including reduced or absent ejaculate, with Duodart
- A blocked or runny nose with Duodart
Get medical help straight away if you notice
- Swelling of the lips, tongue or throat, or difficulty breathing
- A rash with peeling or blistering skin
- Fainting, or dizziness severe enough that you cannot stand safely
- A lump, pain or nipple discharge in the breast tissue
- An erection that will not subside, which is a surgical emergency
Not suitable for everyone
- Women, children and anyone under 18
- Anyone who has reacted to dutasteride, tamsulosin, other 5 alpha reductase inhibitors or soya and peanut
- Men with severe liver impairment
- Duodart specifically: men with a history of fainting on standing, or already taking another alpha blocker
- Duodart specifically: tell your eye surgeon before any cataract or glaucoma surgery, because tamsulosin is linked to intraoperative floppy iris syndrome
Tell the doctor about every medicine you take, including blood pressure tablets, other alpha blockers, ketoconazole, ritonavir, verapamil, diltiazem and any erectile dysfunction medicine, since several of these interact with tamsulosin.
What to expect, and when
The two ingredients work on different timescales, which is the main reason a doctor may start one rather than the other.
WEEK 1 TO 2
The tamsulosin part, if you are on Duodart
Flow and urgency often ease within the first week or two. Dizziness on standing is most likely now. Avodart alone will not do this.
MONTH 1 TO 3
Early prostate change
Prostate volume begins to fall. Symptom scores usually shift only slightly at this stage, so do not judge the medicine yet.
MONTH 3 TO 6
Measurable improvement
Most of the flow benefit from dutasteride appears here, and the PSA reading settles at about half its previous level.
MONTH 6 AND BEYOND
Review with your doctor
The benefit continues only while you keep taking it. Your doctor may review whether the tamsulosin part is still needed, or whether it should be added.
