Daily contraceptive pill ยท Drospirenone

Drospirenone pill in Malaysia

Drospirenone is the progestogen in the daily contraceptive pills Yaz and Yasmin, both made by Bayer. It is paired with an oestrogen, ethinylestradiol, and taken as one tablet a day to prevent pregnancy. Which of the two suits you depends on the oestrogen dose and the routine you prefer, and that is a decision to make with a doctor.

Two routines: 21 and 7, or 24 and 4

Both pills are built around a 28-day cycle. The difference is how those 28 days are split between active tablets and a break. Switch between the two to see a full pack laid out day by day.

  1. Day 1: Active tablet
  2. Day 2: Active tablet

Yasmin: 21 tablets then 7 days without tablets. Your withdrawal bleed usually comes during the break. You must remember to restart on the right day.

Yaz: 24 active tablets then 4 inactive ones, so you take a tablet every single day and never count a break. The shorter hormone-free interval is the difference in the routine.

Neither routine is better than the other. It is about which one you will find easier to keep to, and what your doctor advises for you.

Drospirenone price in Malaysia

Both Yaz and Yasmin are a 28-day supply, so the pack price is the monthly cost and the two are directly comparable.

What the drospirenone pill does

Drospirenone is a progestogen, and in Yaz and Yasmin it is combined with the oestrogen ethinylestradiol. Together they mainly work by stopping ovulation, with additional effects on cervical mucus and the lining of the womb.

Drospirenone is structurally related to spironolactone, which gives it anti-mineralocorticoid and anti-androgenic properties. In plain terms, it is less likely to cause fluid retention than some older progestogens, and it can affect potassium levels, which is why some medicines and kidney conditions need checking first.

It does not protect against sexually transmitted infections. It is taken continuously, so it needs to be dispensed and reviewed on an ongoing basis.

Other reasons people ask about the drospirenone pill

People often ask whether these pills help with acne or with severe premenstrual symptoms. It is a fair question, and you will find a lot written about it online.

In some countries Yaz carries additional licensed indications beyond contraception. What a medicine is licensed for differs from country to country, and in Malaysia the indication registered here is the one that applies.

If you are asking about these pills for a reason other than contraception, that is a conversation to have with a doctor, who can tell you what is appropriate and licensed for you here.

Safety and who should not take it

Blood clot risk with drospirenone pills

Malaysia's National Pharmaceutical Regulatory Agency has issued guidance to healthcare professionals on the risk of venous thromboembolism, or blood clots, with combined oral contraceptives containing drospirenone, naming Yasmin and Yaz.

The regulator noted that published studies found a higher risk of VTE with drospirenone-containing pills than with pills containing levonorgestrel, and that the risk appears similar to pills containing desogestrel or gestodene. The overall risk of a clot on any combined pill remains low, but it is not zero, and it is higher than on a pill that contains no oestrogen.

This is exactly what the doctor consultation is for. A doctor will go through your history and tell you whether this is a suitable choice for you, or whether a different method fits better.

Common side effects

  • Headache
  • Breast tenderness
  • Nausea

Yasmin

Yasmin is the original of the two. It has the higher oestrogen dose, 30mcg of ethinylestradiol, alongside 3mg of drospirenone. It comes as a traditional 21-day pack: you take one tablet a day for three weeks, then have a week without tablets, when a withdrawal bleed usually happens. On day eight after the last tablet you start the next pack, whether or not the bleed has finished. It suits people who are comfortable counting a break and restarting on time.

Read the full Yasmin guide

Yaz

Yaz has the lower oestrogen dose, 20mcg of ethinylestradiol, with the same 3mg of drospirenone. It uses a 24/4 routine: 24 active tablets followed by 4 inactive ones, so there is a tablet to take every day and no break to count. When the strip ends, the next one starts the following day. It suits people who find a daily habit with no gaps easier to keep, and a shorter hormone-free interval is part of the design.

A lower oestrogen dose is not automatically the better choice. Bleeding patterns and side effects vary between people, and a doctor will help you weigh it up.

About the drospirenone pill

Drospirenone is a synthetic progestogen, a man-made version of the hormone progesterone. In Malaysia it is used in combined oral contraceptive pills, where it is paired with the oestrogen ethinylestradiol. It is taken as one tablet a day and mainly works by stopping ovulation.

Drospirenone is the ingredient. Yaz and Yasmin are two brand name pills made by Bayer that both contain 3mg of drospirenone. They differ in the amount of ethinylestradiol they contain and in how the 28-day pack is set out.

Price and getting it

On Hello Doktor, Yasmin is RM57 and Yaz is RM66 for a 28-day pack, which is one month of pills. The doctor consultation is RM1.

Yes. Both are prescription-only medicines in Malaysia. A doctor must assess your health history and issue a prescription before either can be dispensed. You can do this through an online consultation with a female doctor.

Taking it

Safety and side effects

For most people it is a suitable choice. NPRA has advised that drospirenone pills carry a higher risk of blood clots than pills with levonorgestrel, similar to pills with desogestrel or gestodene. The overall risk stays low, and a doctor checks your history before prescribing.
