What the levonorgestrel pill does
Levonorgestrel is a synthetic progestogen, a laboratory made version of a hormone your body already produces. It has been used in contraception for decades, and in an emergency pill it is given as a single 1.5mg course rather than a daily dose.
As an emergency pill it mainly works by delaying or preventing ovulation. If no egg is released, sperm already present cannot fertilise one. It does not end an existing pregnancy and it is not an abortion pill.
It does not protect against sexually transmitted infections, and it is not meant to replace regular contraception. If you find yourself needing it often, that is worth a conversation with a doctor about a method that suits your routine better.

Postinor-2
Postinor-2 is two 0.75mg tablets that together make one 1.5mg course. The Malaysian product information directs taking the first tablet as soon as possible and the second tablet 12 hours later, both within 72 hours of unprotected sex. It is the lower priced of the two packs, and the trade off is remembering the second tablet at the right time. Setting an alarm on your phone the moment you take the first one is the simplest way to avoid missing it.
Escapelle
Escapelle is one 1.5mg tablet taken once, as soon as possible and within 72 hours of unprotected sex. The full course is done in a single swallow, with nothing to remember afterwards and no second dose to time. It costs more per pack than Postinor-2, and for many people that buys peace of mind, particularly if you are travelling, working a long shift, or simply do not want one more thing to keep track of.

Taking levonorgestrel
Take it as soon as possible
Within 72 hours of unprotected sex. With or without food. Swallow with water. Postinor-2 is two tablets 12 hours apart, and Escapelle is one tablet.
If you vomit
If you vomit within 3 hours of taking it, the dose may not have been absorbed. Speak to a doctor or pharmacist about taking another dose.
Afterwards
Your next period may come earlier or later than usual. Take a pregnancy test if your period is more than 7 days late, lighter than usual, or unusual. Use condoms until your next period, and speak to a doctor about starting regular contraception.
Is levonorgestrel safe? Side effects and who should check first
Common side effects
- Nausea
- Headache
- Lower abdominal pain
- Breast tenderness
- Tiredness
- Dizziness
- Spotting or changes to your next period
Most settle within a few days.
Speak to a doctor before taking it if you:
- Take medicines that can reduce how well it works, such as some epilepsy, HIV or tuberculosis medicines, griseofulvin, or St John's wort
- Take ciclosporin
- Have taken ulipristal acetate (Ella) in the last 5 days
- Have severe liver disease, or a bowel condition such as Crohn's disease that affects absorption
- Have had an ectopic pregnancy or a fallopian tube infection before
- Weigh more than 70kg, as a doctor may advise a different option
- Are breastfeeding and have questions about timing
Get medical help urgently if:
- You have severe lower abdominal pain, especially on one side
- You have heavy bleeding
- You faint
These can be signs of a pregnancy outside the womb, which needs to be checked the same day.
