Cialis is the brand name for tadalafil, a prescription medicine in the PDE5-inhibitor class used to treat erectile dysfunction (ED) in adult men. In Malaysia, Cialis is marketed by Eli Lilly and registered with the National Pharmaceutical Regulatory Agency (NPRA). Side effects

Common
Headache, facial flushing, blocked nose, indigestion (dyspepsia), back pain, muscle aches, dizziness.
Less common
Blurred vision, light sensitivity, low blood pressure, uncomfortable heartbeat.
Rare but serious: get help immediately
- Painful erection lasting more than 4 hours (priapism).
- Sudden loss of vision in one or both eyes (NAION).
- Sudden hearing loss or ringing in the ears.
- Severe allergic reaction: swelling of face/lips/tongue, rash, difficulty breathing.
- Chest pain during sexual activity.
Contraindications
DO NOT take tadalafil if you:
- Take any form of nitrate (e.g. glyceryl trinitrate/GTN, isosorbide) or nitric-oxide donor (e.g. nicorandil, amyl nitrite/"poppers"). This combination can cause a life-threatening drop in blood pressure.
- Take riociguat (for pulmonary hypertension).
- Have severe heart disease that makes sexual activity unsafe, a heart attack in the last 90 days, unstable angina, or a stroke in the last 6 months.
- Have severe liver failure, very low blood pressure (<90/50), or uncontrolled high blood pressure (>170/100).
- Have a history of NAION or PDE5-related vision loss.
- Are allergic to tadalafil.
For kidney impairment, older age, or use of alpha-blockers/antihypertensives, your doctor may adjust the dose.

- Other ED medicines: sildenafil (Viagra), vardenafil (Levitra), avanafil (Stendra).

- Alpha-blockers for BPH or blood pressure: tamsulosin, doxazosin, terazosin, alfuzosin, prazosin.
- Multiple blood pressure medicines (ACE inhibitors, ARBs, beta-blockers, diuretics).
- Strong CYP3A4 inhibitors (ketoconazole, itraconazole, ritonavir, clarithromycin, erythromycin), which raise tadalafil levels.
- CYP3A4 inducers (rifampicin, phenytoin, carbamazepine, phenobarbital, St John's Wort), which reduce effectiveness.
- HIV/hepatitis medicines (protease inhibitors, cobicistat).
- Oral antifungals (fluconazole).
- Severe kidney or liver disease, where the dose may need to be lowered.
USE WITH CAUTION
Food, drink & lifestyle
- Grapefruit juice and grapefruit: avoid. It can raise tadalafil levels in your blood.
- Heavy alcohol (>3 units): higher risk of dizziness, headache, and low blood pressure.
- Traditional or herbal medicine: ginseng, yohimbine, and unregistered "male enhancement" pills may interact or secretly contain tadalafil.
- High-dose L-arginine supplements: discuss with your doctor.
SAFE
Generally safe / no major interaction
- Regular food: can be taken with or without food (a fatty meal doesn't affect tadalafil).
- Coffee and tea in normal amounts.
- Most everyday painkillers (paracetamol, ibuprofen), but always double-check with a pharmacist.

This is called priapism and it's a medical emergency. Go to the nearest hospital emergency department right away. An untreated erection lasting more than 4 hours can cause permanent tissue damage and long-term erectile dysfunction. Don't wait to see if it goes away on its own.
I suddenly lost vision or hearing. What should I do?
Stop taking tadalafil immediately and get medical help right away. Sudden loss of vision in one eye (NAION) or sudden hearing loss are rare but serious side effects. Go to the emergency department or call 999.
What if I feel severely dizzy, faint, or my heart is racing?
Lie down and raise your legs to help blood flow. Avoid standing up suddenly. If the symptoms continue, feel severe, or if you faint, get medical help right away. This is more likely if you're also taking alpha-blockers or other blood pressure medicines.
Can I take chest-pain medicine (GTN) after tadalafil?
NO. Avoid all nitrate medicines (nitroglycerin, isosorbide, GTN spray) and "poppers" for at least 48 hours after your last dose of tadalafil. The combination can cause a life-threatening drop in blood pressure. Tell any doctor or paramedic treating you that you've taken tadalafil.
What if I have an allergic reaction (swelling of face, lips, difficulty breathing)?
This is an emergency. Call 999 or go to the hospital emergency department immediately. Severe allergic reactions (anaphylaxis) to tadalafil are rare but need immediate treatment. Do not take another dose.
Should I worry about severe back or muscle pain that won't go away?
Mild back or muscle pain that clears up within 48 hours is a common side effect. But if the pain is severe, lasts more than 48 hours, or comes with other symptoms (fever, dark urine, muscle weakness), stop taking it and see a doctor.
