Oral antifungal drugs such as terbinafine (Lamisil) and itraconazole (Sporanox) may be prescribed. This can promote the growth of new toenail. The new nail is healthy. You may have to take drugs for 12 weeks but you can only see the result after about 4 months when the nail grows back.
A combination of oral and topical antifungal may give you better an outcome.
However, drugs can cause side effects such as rashes on your skin, nausea, headache, or damage to your liver. During the treatment, doctor may recommend blood tests to make sure the drugs are working properly. If you have liver or heart disease, tell your doctor and he or she can change the prescription to fit your condition.
In severe cases, nail removal is required. The doctor will remove it partially or completely. New toenail can grow at the affected place. However, it may take a few months, even a few years. Your doctor may give you a cream to apply to your nail bed. This can prevent infection

Laser therapy can be used alone or in a combination with medications. Several types of lasers can ber used. This treatment can help the nails improve. However, laser therapy is expensive and the technique is not available in somewhere.
