Uses

What is Choriogonadotropin Alfa used for?

Choriogonadotropin Alfa is used as part of a treatment program for certain fertility problems in women. It is generally used in combination with another hormone (FSH). This medication provides the hormone (hCG), which stimulates the release of a mature egg (ovulation) and helps you become pregnant.

This medication is not recommended for women whose ovaries no longer make eggs properly (primary ovarian failure).

This medication has not been shown to be effective for weight loss and should not be used for this purpose due to risk of serious side effects.

How should I take Choriogonadotropin Alfa?

Inject this medication under the skin (subcutaneously) usually one day after your last dose of the hormone FSH, or as directed by your doctor. Follow your doctor’s instruction closely to ensure proper timing of this medication.

Wash hands with soap and water before using this medication. Before using, check this product visually for particles or discoloration. If either is present, do not use the liquid.

Before injecting each dose, clean the injection site with rubbing alcohol. Inject the prescribed dose as directed by your doctor. Discard any unused portion of the medication immediately after the injection. Do not save it for future use. Do not change the dose unless instructed by your doctor or use this medication for a longer period than prescribed.

How do I store Choriogonadotropin Alfa?

Store in the original container at room temperature or in a refrigerator. If stored at room temperature, throw away any part not used after 30 days. Protect from light. To prevent drug damage, you should not store Choriogonadotropin Alfa in the bathroom or the freezer.

This product may contain inactive ingredients, which can cause allergic reactions or other problems. Talk to your pharmacist for more details.

This medication should not be used if you have certain medical conditions. Before using this medicine, consult your doctor or pharmacist if you have: other fertility problems (e.g., primary ovarian failure), thyroid or adrenal gland problems, cancer of the reproductive organs (e.g., breast, uterus, ovary), tumor in the brain (e.g., pituitary tumor), abnormal uterine/vaginal bleeding, unexplained ovarian cysts or enlarged ovaries.

Before using this medication, tell your doctor or pharmacist your medical history, especially of: blood clots, stroke, certain heart diseases (e.g., heart attacks), liver disease, lung problems (e.g., asthma).

Multiple births may occur because of this treatment. Consult your doctor for more details.

Stop using this medication when you become pregnant. This medication must not be used during pregnancy. If you think you may be pregnant, tell your doctor right away.

It is not known whether this drug passes into breast milk. Consult your doctor before breast-feeding.

Is it safe during pregnancy or breast-feeding?

There are no adequate studies in women for determining risk when using this Choriogonadotropin Alfa during pregnancy or while breastfeeding. Please always consult with your doctor to weigh the potential benefits and risks before taking Choriogonadotropin Alfa. Choriogonadotropin Alfa is pregnancy risk category X according to the US Food and Drug Administration (FDA).

Side effects

What side effects can occur from Choriogonadotropin Alfa?

Redness or pain at the injection site, mild abdominal pain, mood changes, or mild nausea/vomiting may occur. If any of these effects persist or worsen, notify your doctor or pharmacist promptly.

Remember that your doctor has prescribed this medication because he or she has judged that the benefit to you is greater than the risk of side effects. Many people using this medication do not have serious side effects.

Tell your doctor right away if the following unlikely but very serious side effect occurs: unusual vaginal bleeding.

Seek immediate medical attention if any of these rare but very serious side effects occur: weakness on one side of the body, slurred speech, vision changes, sudden/severe headache, pain/swelling of the calf muscles, chest pain, shortness of breath, swelling of ankles/hands/feet.

This medication may cause a condition known as Ovarian Hyperstimulation Syndrome (OHSS). This condition may occur during therapy and after treatment has been discontinued. Rarely, serious OHSS causes fluid to suddenly build up in the stomach, chest, and heart area. Seek immediate medical attention if you develop the following side effects: severe pain or swelling in the lower abdominal (pelvic) area, severe nausea/vomiting, sudden/rapid weight gain, decreased urination.

A very serious allergic reaction to this drug is unlikely, but seek immediate medical attention if it occurs. Symptoms of a serious allergic reaction may include: rash, itching/swelling (especially of the face/tongue/throat), severe dizziness, trouble breathing.

Not everyone experiences these side effects. There may be some side effects not listed above. If you have any concerns about a side-effect, please consult your doctor or pharmacist.

What drugs may interact with Choriogonadotropin Alfa?

A product that may interact with this drug is: gonadorelin.

This medication may affect the results of certain lab tests (gonadotropin levels). Be sure to tell your doctor or laboratory personnel that you are taking this medication.

Choriogonadotropin Alfa may interact with other drugs that you are currently taking, which can change how your drug works or increase your risk for serious side effects. What is the dose of Choriogonadotropin Alfa for an adult?

Usual Adult Dose for Ovulation Induction

Human Chorionic Gonadotropin (HCG): 5000 to 10,000 units IM once 1 day following the last dose of menotropins

Recombinant Chorionic Gonadotropin (r-HCG): 250 mcg subcutaneously once 1 day following last dose of follicle-stimulating agent

Comments:

-Some experts recommended HCG doses of 10,000 units.

-Patients should be appropriately pretreated with human menotropins.

-r-HCG should not be given until adequate follicular development is indicated with serum estradiol and vaginal ultrasonography; treatment should be withheld in patients with excessive ovarian response (e.g., significant ovarian enlargement, excessive estradiol production).

Uses:

-Induction of ovulation (OI) and pregnancy in the anovulatory, infertile woman in whom the cause of anovulation is secondary and not due to primary ovarian failure

-Induction of final follicular maturation and early luteinization in infertile women who have undergone pituitary desensitization and who have been appropriately pretreated with follicle stimulating hormones as part of an assisted reproductive technology (ART) program (e.g., in vitro fertilization, embryo transfer)

Usual Adult Dose for Hypogonadism – Male

HCG:

500 to 1000 units IM 3 times a week for 3 weeks, then 500 to 1000 units IM 2 times a week for 3 weeks OR

4000 units IM 3 times a week for 6 to 9 months, then 2000 units IM 3 times a week for 3 months

Uses:

-Selected cases of hypogonadotropic hypogonadism in males

-Hypogonadism secondary to a pituitary deficient in males

Renal Dose Adjustments

Use with caution.

Dose Adjustments

r-HCG:

-Patients with tubal obstruction: Patients should be enrolled in an in vitro fertilization program prior to treatment.

Signs/symptoms of precocious puberty: Treatment should be discontinued.

What is the dose of Choriogonadotropin Alfa for a child?

Usual Pediatric Dose for Prepubertal Cryptorchidism

HCG:

4 years and older:

4000 units IM 3 times a week for 3 weeks OR

5000 units IM every other day for 4 injections OR

500 to 1000 units IM for 15 injections over a period of 6 weeks OR

500 units IM 3 times a week for 4 to 6 weeks; if unsuccessful, patients should be given an additional series using 1000 units starting 1 month later.

Comments:

-Treatment is believed to induce a temporary testicular descent response in patients who would have testicular descent during puberty, but may produce a permanent response in some patients.

-Treatment usually occurs between 4 and 9 years of age.

Use: Prepubertal cryptorchidism not due to anatomical obstruction

Precautions

Safety and efficacy of HCG have not been established in patients younger than 4 years.

Safety and efficacy of r-HCG gonadotropin have not been established in children.

How is Choriogonadotropin Alfa available?

Choriogonadotropin Alfa is available in the following dosage forms and strengths:

  • Intramuscular powder for injection,
  • Injectable powder for injection,
  • Subcutaneous powder for injection,
  • Subcutaneous solution.

What should I do in case of an emergency or overdose?

In case of an emergency or an overdose, call your local emergency services or go to your nearest emergency room.

What should I do if I miss a dose?

If you miss a dose of Choriogonadotropin Alfa, take it as soon as possible. However, if it is almost time for your next dose, skip the missed dose and take your regular dose as scheduled. Do not take a double dose.
